Facility construction and renovation projects provide an opportunity to identify and mitigate or eliminate conditions in the health care built environment that may negatively affect the safety of patients, staff, or visitors. This toolkit is intended to guide you through the performance of a safety risk assessment, as required in the 2014 FGI Guidelines for Design and Construction of Hospitals and Outpatient Facilities.

The toolkit was created by the Center for Health Design (CHD) through a consensus process that involved numerous safety risk experts with support from the Agency for Healthcare Research and Quality and the Facility Guidelines Institute.

Beginning in September 2017, the toolkit—previously a PDF—is available as a more functional online tool. To access the free toolkit, please click the link below. Once on the CHD website, create a user ID and password, which will allow you to save projects in the program.
